The Legislative Breakfast provides an opportunity to inform                                legislators about the needs and accomplishments                                                         of our family members with a disability.

                  Service providers and other professionals are urged to attend.                   There is a fee of $25 per person. Make checks payable to Angela's House.

                 You must register by mail. Print and complete the form below                                 and mail it along with your check to the address shown.

                 Checks must be received no later than September 26, 2008.

                 Pre-registration is a must. If an agency is bringing consumers,                        the consumers must be registered.
